SCHEDULING A NEW PATIENT APPOINTMENT
Dr. Livstone sees patients with digestive problems who are 8 years old and older. To schedule a new patient appointment, call the main office line (941) 955-0000. To make sure we understand your medical history, your physical condition, and the nature of your problem before undertaking any diagnostic or treatment plans, we usually schedule a 1 hour appointment for a new patient visit. Our receptionist will ask your name, address, telephone number, general nature of the problem, and your medical insurance information; we are participating providers for Medicare and most managed care plans (HMO's) as well as most Preferred Provider Organizations (PPO's) and commercial insurance plans. We also see patients who are self-insured. We are not participating providers in the Medicaid and Workmen's Compensation programs. We also do not participate in Medicare HMO plan or Medicare Advantage plans.
When you are given a new patient appointment, the receptionist will inquire whether you want us to mail a REGISTRATION FORM and a MEDICAL HISTORY FORM to you as well as directions to the office. If you want to receive these forms more quickly and to complete them at your leisure before your first visit, you may download these forms from this web site. Print the 2 forms, fill them out, and bring them with you to the office at the time of your first visit. You can also obtain directions to the office by using the Google maps link on this website.
If you choose to have the medical history and registration forms mailed to you, complete the mailed forms and bring them with you to the office at the time of your first visit. In that instance, when you come to the office for your first visit, we will give you our NOTICE OF PRIVACY PRACTICES, a SURROGATE-TELEPHONE AUTHORIZATION form, and a CONSENT FOR OUR PRIVACY PRACTICES form. The execution of these privacy forms is required by the Federal Privacy Act which went into effect April 14, 2003.
If you prefer, you may view our one page ABRIDGED NOTICE OF PRIVACY PRACTICES or the nine page COMPLETE NOTICE OF PRIVACY PRACTICES on this web site, then download the SURROGATE-TELEPHONE AUTHORIZATION form and the CONSENT FOR OUR PRIVACY PRACTICES form from this web site, complete them at your leisure before coming to the office, and bring them with you at the time of your first visit. If you complete all of these forms before coming to the office, you can be seen immediately upon your arrival at the office. The receptionist will be glad to help you with any questions you might have about these documents.
